I was listening to that shit earlier today and realized he was talking about a rapper who was fallin off. Is that just a concept for the song or is he talkin about someone in particular?

I think he's talking about himself, from the perspective of his old self to the person he is now. He's not saying he fell off, just that he's changed and he sometimes forgets his struggle.
